Standardizing LOA templates - what works?

Been in business for four years now and still tweaking my LOA template. Wondering what key provisions others have found essential. Currently using a pretty basic contingency agreement but thinking I need more comprehensive language around scope, payment terms, and client responsibilities. What are the must-have clauses that have saved you headaches down the road?
Jennifer, one thing I learned working with Rocky Mountain Power accounts here in Utah - always include specific language about what constitutes 'completion' of your services. I used to have disputes about whether finding errors was enough or if I had to shepherd the refund process through to the end. Now I clearly define deliverables and what triggers payment. Saves a lot of arguments later.
Connie's right about defining completion. I also always include a clause about client cooperation - requiring them to provide all requested documentation within 30 days and to sign utility authorization forms. With APS accounts here in Phoenix, I've had clients drag their feet on providing bills, then complain about project delays. Now I have clear timelines and consequences for non-cooperation.
Sarah makes a great point about client cooperation. Another clause I consider essential - intellectual property ownership. Your analysis methodology and findings are your work product. I learned this when a client in Connecticut tried to use my detailed report to negotiate directly with other vendors. Now I retain all rights to my work papers and methodologies, and clients only get summary reports of findings.
