Red flags that make you dig deeper?

What are the red flags that make you spend extra time on an account? Things that just don't look right and usually indicate bigger problems?
Manny - sudden changes in demand charges without corresponding usage changes. Found a faulty CT that was reading 3x actual usage, costing the client $400/month extra for 8 months.
Inconsistent power factor charges are always suspicious. Either the customer's equipment changed dramatically or the utility is calculating wrong. Usually worth investigating.
When the billing determinant doesn't match the rate schedule being applied. Caught MLGW billing a small office on an industrial rate for 2 years. $15K refund.
Good list everyone. I'd add bills that show estimated reads for multiple consecutive months. Usually means meter access issues, but sometimes indicates meter failure that's being covered up.
Perfect timing kWh that always hits exact tier breakpoints. Real usage has variation - when it's too perfect, the meter or billing system might have issues.
